Make sure the file is of good quality.

Please ensure the following information is clearly indicated on the request:

• Your full name
• Your date of birth or health insurance number
• A phone number where you can be reached
• The requested exam must be clearly specified
• Doctor’s signature
• The requester’s license number must be legibly written

We offer our clients free parking (conditions apply) in the underground parking lot of the Université de Sherbrooke, accessible via Saint-Charles Street. Follow “Impasse du métro” on Google Maps. Our state-of-the-art clinic is located on level B1, ground floor, for easy access.

Patients coming for exams covered by RAMQ must pay for their parking.
